Report an Accommodation Concern
Students with issues receiving accommodations or trouble resolving accommodation-related issues with faculty or staff can submit a written statement to the Office of Student Accessibility Services (SAS). This procedure allows the Office of Student Accessibility Services to provide support and advocate on the student’s behalf. To begin the process, send a report to the SAS department email, accessibility@okstate.edu. Your report should include the following:
- Name of the person or department involved.
- Course name and number if the situation involves a professor.
- Date the situation occurred.
- If relevant, the location of the incident/situation.
- A brief synopsis of what occurred.
- The desired outcome. (Example: Retake an exam with requested accommodations)
Any accommodation complaints for other OSU campuses or for matters unrelated to accommodations or accommodation implementation should be directed to the appropriate party, including but not limited to the Department Head, Dean, Student Conduct, or the Office of Equal Opportunity.
The Process
- Student Accessibility Services will first encourage the student to resolve disputes at the informal level by speaking directly to the staff/faculty/parties involved.
- Should the student not get a response from the staff, faculty, or parties involved, Student Accessibility Services will attempt to contact the staff/faculty/parties involved to discuss the student’s complaint.
- If accommodations are not being implemented appropriately and/or a resolution cannot be achieved, SAS will contact the professor and head of the department to address the discrepancy.
- If, after discussion with the department head, a resolution is still not achieved, or if the student is not satisfied with the resolution, the student’s complaint is elevated to “grievance” status. The student will then be directed to contact the Office of Academic Affairs to complete a formal Academic Affairs Grievance form and to be advised regarding further steps that will be taken.
